Cognito Forms is a flexible, technically capable online form builder with a strong template library and a WordPress plugin, offering HIPAA compliance (and a BAA) on its Enterprise service level. Therapists use it for custom intake, consent, and screening forms with conditional logic, calculations, payments, and e-signatures, often at a lower cost than enterprise incumbents. Not therapy-specific, and HIPAA is gated to the Enterprise tier - confirm the BAA before collecting any PHI. A good middle-ground between basic form tools and full intake platforms.
